1. By registering with Rotary Hockey you agree to all rules, regulations, bylaws and constitutions. These documents can all be found on this website.
2. Rotary Hockey will not guarantee any player / team or coaching assignments.
3. Rotary Hockey shall have the authority to reassign individual players to a different division or to a team within their own division to equalize the competitiveness of teams across the league.
4. I certify that the information provided to be true and in consideration of the granting of this registration to me with the privileges incident thereto, and by registering I have become subject to and agree to abide to the rules, regulations and decisions of Rotary Hockey and Hockey Canada, its Board of Directors, its Members and/or associations. Further, the information to be provided is required by Hockey Canada to facilitate hockey programs on behalf of the registrant and Hockey Canada. Rotary Hockey and Hockey Canada will treat this personal information with the utmost respect and in accordance with the Hockey Canada Privacy Policy at all times.
5. I acknowledge that photos taken in open public spaces during games and/or practices at the arenas may be used in promotional or social media posts and permission is hereby granted to Rotary Hockey to use such images.
6. Participation in any sport or other activity brings with it a risk of injury or loss. Rotary Hockey is not responsible for any claim, loss, injury or damage to persons or property suffered during activities both on and off the ice.
7. ARENA ZERO TOLERANCE POLICY - The City of Stratford and the Minor Sports Council Members: Physical or verbal abuse directed towards officials, convenors, coaching staff, arena staff, players or spectators before, during or after a sports function will not be tolerated. Offending person(s) may be asked to vacate the sports venue(s). A further banning may result pending review by participating sports group(s).
8. HOCKEY CANADA - Use of Personal Information Waiver - Hockey Canada does not sell, trade or otherwise share the personal or contact information we collect outside our Members and Associations. However, we may from time to time use this information for the purposes of offering you additional services, notifying you about promotions (including promotions offered by third parties or sponsors), and/or to conduct hockey specific research. Such communications may come from Hockey Canada, its Members and/or Associations, or may come directly from the third parties/sponsors. This type of usage of your personal or contact information by Hockey Canada, its Members, its Associations and or third parties/sponsors is entirely at your discretion. Should you choose to allow this type of usage, please check the box below.
9. HOCKEY CANADA - Acknowledgement - I certify the information provided to be true and in consideration of the granting of this registration to me with the privileges incident thereto, and by registering I have become subject to the rules, regulations and decisions of Hockey Canada, its Board of Directors, its Members and/or associations which may be restrictive in some areas such as movement from team to team, conduct etc. and I agree to abide by such rules, regulations and decisions of Hockey Canada, its Board of Directors, its Members and/or associations.
10. HOCKEY CANADA/ALLIANCE HOCKEY - Live Streaming Consent and Release - This season Alliance Hockey games will be livestreamed. Live video of Alliance Hockey games, tournaments and practices will be recorded and broadcast online.
Recording equipment at hockey games may record your image, voice and other identifying features and personal information. Your behaviour and conduct will be recorded and streamed online.
The location of recording equipment at the arena will be marked by signage. No recording equipment will be installed in areas where participants have an expectation of privacy (e.g. change rooms, washrooms, etc.).
By checking the box below and attending or participating in Alliance Hockey games and tournaments, and practices Participant (and/or Parent/Guardian) are consenting to the recording of the video recordings, which contain your personal information, and to the use of the recordings and personal information as described below.
Use of the Livestream Footage, Images and Sound
Footage or images from the livestream may be used by Alliance Hockey and/or all Alliance Hockey Sanctioned Minor Hockey Associations or their representatives for the following purposes:
training and education purposes, such as skill correction, analysis, etc.;
promotional or marketing purposes, e,g. footage or images may be used on Alliance Hockey and/or Alliance Hockey Sanctioned Minor Hockey Associations website, social media or other promotional material;
commercial purposes, i.e. to advance Alliance Hockey’s commercial or economic interests;
investigatory, complaint management or disciplinary purposes, e.g. footage or images may be used as evidence in any investigation, hearing or disciplinary process brought under Alliance Hockey and/or Ontario Hockey Federation rules and procedures; and,
any other purpose for which Participant (or Parent/Guardian) provide consent.
Disclosure of the Livestream Footage and Images
Footage or images from the livestream will be available publicly on the website: https://www.livebarn.com/en/. It may also be posted to the websites of Alliance Hockey and Alliance Hockey Sanctioned Minor Hockey Associations, as well as their social media platforms.
The livestream video and images will not otherwise be disclosed by Alliance Hockey or Alliance Hockey Sanctioned Minor Hockey Associations without Participant’s (or Parent/Guardian’s) consent, or if required to do so by law, a court order, a regulator, a law enforcement agency, or during legal proceedings.
Consent and Release
I give permission for my (and/or the Minor Participant's) image or physical likeness (both in video and photo), name and/or personal information to be featured on the livestream and used for the purposes set out in this form.
I relinquish (and/or relinquish on behalf of the Minor Participant) any rights, title or interest that I (and/or the Minor Participant) may have in the livestream footage, images or sound.
I agree that (the Minor Participant and/or) I have no rights to fees, remuneration or royalty in exchange for my (or their) appearance in the livestream footage or images.
I further agree that I (and/or the Minor Participant) waive all rights that I (or they) may have to control the use of the livestream footage or images.
I agree (and/or agree on behalf of the Minor Participant) that Alliance Hockey will be the sole owner of all rights, title and interest in the livestream images and footage, and have unrestricted and perpetual rights to use the images, footage or sound for the purposes set out above.
I agree to indemnify Alliance Hockey and/or Alliance Hockey Sanctioned Minor Hockey Associations, their employees, service providers, licensees, successors, legal representatives, agents and assigns with respect to any damages, costs, expenses, liabilities, fines, penalties, or other losses suffered by any of them in regard to any challenge by Participant and/or any Parent/Guardian in regard to the rights conferred to Alliance Hockey and/or Alliance Hockey Sanctioned Minor Hockey Associations hereunder and/or the exercise thereof by Alliance Hockey and/or Alliance Hockey Sanctioned Minor Hockey Associations.
I represent and warrant that if this Consent and Release is given on behalf of a Minor Participant, that any other Parent/Guardian has been informed of the terms of the Consent and Release, and that they agree to them.
I consent to the collection, use and disclosure of the livestream footage, video and audio (on my behalf, on behalf of the Minor Participant and on behalf of any other Parent/Guardian) and agree to the statements above.
