Active Screening for COVID at all Arenas
The City of Stratford will continue to actively screen all those who enter the arenas even as we have moved into the Yellow - Protect Zone. This means that you will need to fill put and submit the screening questionnaire for your family as you enter the facilities.
If you would like to prefill the form so that you do not have to fill it out in the lobby, you can download and print a copy from the links below:

The City will also allow you to take an extra copy home with you from the arena so that you can bring it back next time you visit the arena. Please only take one extra form at a time.
